This 8″ x 5 5/8″ Individual Perpetual Membership folder is available for a donation of $15 (per person) or $25 (per couple). Premium Individual Perpetual Membership

For an offering of $20 (per person) or $30 (per couple), the 8-1/2″ x 6-1/2″ Premium Individual Perpetual Membership folder is available with a picture of Our Lady of Grace or Our Blessed Mother with Roses. Deluxe Individual Perpetual Membership
For an offering of $25 (per person) or $35 (per couple), you can request a Deluxe Individual Perpetual Membership certificate and folder. The deluxe certificate is mounted in an 10-1/2″ x 8-5/8″ slate-blue leatherette folder. A picture of Our Lady of Grace completes the inside of the folder opposite the membership certificate. Family Perpetual Membership
Each Family Perpetual Membership comes with an engraved certificate in a deluxe 12 1/4″ x 9″ folder.
You may request a Family Perpetual Membership for an offering of $50. This membership includes the husband and wife, their children and grandchildren. It also includes the parents, brothers, and sisters of both the husband and the wife. The family name on the certificate will be printed in gold.
